Simmonds showed a passion for acting at a very young age. Before landing her breakthrough role, she participated in school plays. She had performed at the Utah Shakespeare Festival and was in a deaf student's short film Color the World.
Millicent Simmonds auditioned for a role in the film Wonderstruck and landed the role of Rose, beating over 250 others. She was recognized as one of eight actors who were Breakthrough Entertainers of 2017 by The Associated Press.

Since her breakthrough role, Millicent Simmonds has portrayed the role of Regan Abbott in A Quiet Place (2018) and Quiet Place: Part II (2020).
